		Plan the hospital discharge Arrange careful transportation to the Elmbrook nursing home of your choice. (We have eight in the southern Oklahoma area to choose from!) Select a schedule of speech, physical and occupational therapies necessary for quick rehabilitation and return home Coordinate diet plans Create a home-like environment that will nurture healing and eventual independence
Our staff will also work with you to arrange payment from a variety of payment options. Medicare will pay for Skilled Nursing benefits if the patient is enrolled in Part A (hospital insurance) and meets additional requirements. Other payment options include long-term care insurance, private insurance, private pay and others. Treatments available: Physical Therapy including assistance with hip fractures, joint replacements, strokes, traumatic brain injury, mobility deficits with Alzheimer's/Dementia. Occupational Therapy which enables residents to return to the activities of daily living such as self-care, home maintenance and leisure activities.
